Health Care

The health care assistance is provided by way of monetary assistance to individuals suffering from cancer, cardio-vascular diseases, nephrological ailments, tuberculosis, neurological diseases, or any life threatening or debilitating diseases and requiring hospitalization or treatment over a long duration and/or incurring heavy expenditure towards the same.

Eligibility criteria

Actual verification of socio-economic back ground (by referring following documents)
- Income Certificate
- Other criteria as mentioned in the application form
Recommendation from Local Panchayath/Municipality/corporation member/President/Chairman as the case may be
Copy of following documents:-
- Voter’s ID
- Aadhar Card
- Ration card (relevant pages)
Original letter from consulting doctor stating the details of illness
Latest bill of medicine purchased
If the help is for organ transplantation, relevant sanction letter from medical board authorities
